Overview

Are you a Certified Nursing Assistant (CNA) with Skilled Nursing Facility (SNF) experience who thrives on building relationships and making a difference in the lives of seniors? At Gentiva Hospice, we believe every day is an opportunity to bring comfort, dignity, and peace to patients and families. We're currently seeking a compassionate, dependable CNA to serve patients in skilled nursing facilities throughout the Dallas area and act as a liaison between Gentiva and facility staff to ensure quality, coordinated care.

What

You'll Do
  • Provide personal care and support to patients residing in SNFs.
  • Assist with daily activities including hygiene, grooming, mobility, and comfort needs.
  • Observe and report changes in patients' conditions to the care team.
  • Build strong, trusting relationships with facility staff, residents, and families.
  • Support facilities in coordinating hospice services and care plans.
  • Represent Gentiva Hospice at facility meetings and help educate staff about hospice philosophy.
  • Maintain timely, accurate documentation and uphold safety standards.
About You Qualifications – What You'll Bring
  • Current CNA certification (Texas) with experience in Skilled Nursing Facilities.
  • Compassion, professionalism, and strong interpersonal skills.
  • Ability to collaborate effectively with both facility staff and hospice team members.
  • Reliable transportation for travel to assigned facilities.
  • Prior experience in hospice or palliative care a plus, but not required.
Preferred Background (Not Required)
  • Experience in home health, hospice, palliative care, or oncology.
  • Prior roles in hospital nursing, ICU, geriatrics, med-surg, ER, telemetry, or nursing homes.
  • Exposure to end‑of‑life care, wound care, or admissions support.
