We are seeking a motivated and compassionate Licensed Vocational Nurse (LVN) to join our home health and hospice team.
PositionSummary
The LVN provides skilled nursing care to adult patients in their homes; coordinates care with the interdisciplinary team and serves as a primary point of contact under the Director of Nursing. This role requires strong clinical judgment, excellent communication skills, and a passion for patient advocacy.
Responsibilities- Provide skilled nursing care to adult home health and hospice patients
- Perform patient assessments and develop individualized care plans
- Coordinate care with physicians, families, and interdisciplinary team members
- Educate patients and caregivers on diagnoses, medications, and care plans
- Maintain accurate and timely clinical documentation
- Participate in on-call rotation and respond to PRN/emergency visits as needed
- Current Texas LVN license (required)
- Minimum 1 year of home health or hospice experience preferred
- Current CPR certification (required)
- Valid Texas driver’s license, reliable transportation, and auto insurance
- Ability to communicate effectively in English
- Hepatitis consent/declination documentation
- Strong interpersonal and organizational skills
- Marketing experience or community outreach interest is a plus
- Ability to stand, walk, and lift patients as needed
- Comfortable working in varying home environments
- Potential exposure to blood-borne pathogens
- Flexible with local travel and scheduling
