Job Summary and Qualifications
* This is a Full Time Day shift position, Monday - Friday 8:00am - 4:00pm with some shared On-Call requirements *
The Heart Failure/VAD Coordinator is a registered nurse who plans, coordinates and implements the care of the heart failure patient and the pre‑ and post‑VAD patient, as applicable, in both the in‑patient and out‑patient settings in accordance with the nursing process. This will include Case Management duties as required.
What You Will Do In This Role:The scope of the role includes acting as a liaison with the heart failure patient population, the pre‑transplant Heart Transplant Coordinator, the VAD Program Supervisor and community care providers in order to match patient clinical needs to program/procedural offerings.
Coordinating information flow among medical, ancillary, nursing, financial and insurance company staff
Representing the heart failure, VAD and transplant program(s) through involvement in hospital and community activities
Coordinator participates in the development and maintenance of the transplant/VAD statistical databases, Intermacs and any registries pertaining to VAD services.
Supports the VAD Process Improvement Committee preparations as required.
Supports requirements for continued VAD Certification by Joint Commission.
Educates the patient and family, staff and community as required
· Manages equipment, supplies, and charges in conjunction with hospital
- Graduate of accredited school of professional nursing
- BSN required within 24 months of hire
- Current RN license in the State of TX or compact license
- Current BLS issued by the American Heart Association or the American Red Cross
- ACLS required within 6 months of hire issued by the American Red Cross or the American Heart Association
- Two years critical care experience
- One year experience in management of organ failure/transplant/VAD patients preferred
- Shares on-call requirements to support program
