Job Summary

The primary function of the Hospice LVN is to administer skilled nursing care, under the supervision of a Registered Nurse, for clients of all ages in their place of residence. You will coordinate care with the interdisciplinary team (IDT), patients, families, and referring agencies to ensure a holistic approach to end-of-life care.

Key Responsibilities
  • Clinical Care: Supplement nursing care needs as specified in the Plan of Care (POC) and assist the RN or physician in performing specialized duties related to end-of-life care.
  • Patient Advocacy: Assist patients and families in learning appropriate self-care techniques and assess their response to care, ensuring all pertinent findings are communicated to the team.
  • Documentation: Complete appropriate clinical and progress notes, accurately documenting the outcomes of interventions and patient status changes.
  • Treatment Preparation: Prepare equipment and materials for treatments, strictly adhering to aseptic techniques and organizational policies.
  • Collaboration: Communicate effectively with the IDT regarding changes in patient conditions to ensure seamless coordination of care.
  • Quality Assurance: Promote Agency philosophy and administrative policies to ensure the gold standard of clinical excellence.
Qualifications
  • Education: Graduate of an accredited school of vocational nursing.
  • Licensure: Current State of Texas license as a Licensed Vocational Nurse (LVN).
  • Certification: Proof of current CPR certification is required.
  • Experience: Two (2) years of experience as an LVN in a clinical care setting; hospice or home health experience is preferred.
  • Skills: Strong nursing skills as defined by generally accepted standards of practice; excellent interpersonal skills; must be able to read and write English.
  • Requirements:
    • Valid Texas Driver’s License and proof of valid Texas Automobile Liability Insurance.
    • Proof of TB test and Hepatitis consent/declination.
    • Ability to lift up to 50 pounds and move patients.
Working Conditions

This position involves local travel to visit patients in their homes in various conditions. You should be comfortable with a flexible schedule and potential exposure to blood, bodily fluids, infectious diseases, and unpleasant weather. The role requires prolonged standing, walking, hand-eye coordination, and the ability to make quick clinical decisions under stressful conditions.
