Job Summary

As an RN Clinical Nurse Liaison, your primary mission is to act as the face of VNA in the community, developing and promoting hospice services while ensuring clinical standards are met. You will handle General Inpatient (GIP) and routine admissions, acting as the primary RN contact for hospital-based transitions and ensuring a seamless journey for patients and their families.

Key Responsibilities
  • Community Liaison: Build and maintain effective working relationships with physicians, community agencies, and the public to foster a deep understanding of VNA services.
  • Clinical Admissions: Perform GIP and routine admissions in hospitals, facilities, or homes; evaluate patients for hospice criteria in collaboration with physicians.
  • Strategic Planning: Partner with the Director of Marketing and Chief Nursing Officer to analyze metrics, update strategic plans, and document activities within Salesforce.
  • Compliance & Advocacy: Ensure all marketing and clinical activities comply with local, state, and federal laws, while serving as a clinical resource for the Interdisciplinary Team (IDT).
  • Public Relations: Establish and maintain a public relations program to interpret agency services for the professional and lay public.
Qualifications
  • Education: Graduate of an accredited college or university is preferred;
    current and verifiable Texas RN licensure is required
    .
  • Experience: Proven experience in the healthcare arena;
    hospice experience is strongly preferred
    .
  • Skills: Strong ability to establish professional relationships, excellent communication skills, and proficiency in timely, compliant documentation (Texas BON standards).
  • Requirements: Valid Texas Driver’s License, reliable transportation, and current auto liability insurance.
    • Proof of CPR certification, TB testing, and Hepatitis acceptance/declination.
    • Ability to pass a criminal background check.
Working Conditions

This role requires significant local travel and a flexible schedule to meet the needs of the community and referring facilities. You must be able to handle stressful situations with a calm, courteous demeanor and work effectively in both office and facility environments. The position involves sitting for periods of documentation and navigating various weather conditions during travel.
