Job Summary
This position works with limited oversight to efficiently provide nursing triage of all patient messages received via telephone or MyChart. The triage nurse demonstrates sound clinical judgment that utilizes careful listening, critical thinking and assessment skills. During downtime the position processes patients to appropriate units based on physicians' orders whether coming via ED, DA, Transfer, Clinic, Surgery or Procedural Area.

Certifications:
RN – Registered Nurse licensure as Registered Nurse by Board of Nurse Examiners for the State of Texas
- 2 years of ambulatory clinical nursing experience or emergency room experience
- Utilizes strong listening skill to interpret subtle indications of the patient’s complaint to form a thorough assessment without visual observation.
- Follows clinic‑specific protocols and professional judgment to identify nature and urgency of patient needs to determine appropriate disposition.
- Provides patient education and advice to improve continuity of care, avoid unnecessary ER visits and reduce risks from medical complications.
- Collects sufficient data related to presenting problem and medical history and recognizes symptom patterns to those in clinic protocols to provide safe, timely disposition.
- Refills medications per clinic‑specific protocols.
- Establishes and maintains ongoing communication with internal stakeholders and external customers while securing patient admission or transfer.
- Applies clinical expertise and the nursing process to triage and prioritize admission acuity from multiple locations: direct admits, external transfers, ED, onsite clinics and private physician offices.
- Ensures smooth functioning of the patient placement process to reduce wait times and avoid unnecessary movement of patients, serving as an expert clinical resource for patient placement.
- Facilitates the direct admit process by interacting with physicians to ensure telephone triage results in appropriate patient placement.
- Notifies Administrative Coordinator or Manager concerning admissions that do not meet hospital criteria.
- Continually seeks new ways to improve processes and increases efficiencies, communicating recommendations to the Administrative Coordinator or Manager.
- May perform core functions such as directly interacting with or caring for patients, maintaining or releasing patient records, or other duties as assigned.
