Job Overview

We are seeking a Senior Community Care Nurse Practitioner (NP) to conduct comprehensive assessments and support patients in nursing homes, senior housing, and assisted living settings. The NP will participate in patient visits following hospitalization to ensure a smooth transition from rehab to home and prevent readmission.

The role offers a $3,500 sign‑on bonus for external candidates. This NP will capture STAR measures, educate patients, and document diagnoses, while communicating findings to attending physicians and facility staff. The NP will not be responsible for orders, treatments, or follow‑up care.

Primary Responsibilities
  • Perform comprehensive assessments and document findings in a concise and regulatory‑compliant manner, adhering to CMS documentation requirements.
  • Ensure all diagnoses are coded accurately in ICD-10 to support the visit.
  • Address and document all required quality elements.
  • Conduct an initial medication review.
  • Attend and complete all mandatory educational and computer/virtual training requirements.
  • Travel between care sites as required.
  • Care Coordination
    • Communicate with patients and required team members regarding any medical concerns during the visit.
  • Program Enhancement Expected Behaviors
    • Exhibit original thinking and creativity in developing new and improved approaches to concerns/issues.
    • Function independently and responsibly with minimal supervision.
    • Enter available hours into the web‑based application at least one month prior to work time.
    • Demonstrate initiative in achieving individual, team, and organizational goals and objectives.
    • Participate in SCC quality initiatives.
    • Check Optum email intermittently for required trainings, communications, and monthly scheduling.
Required Qualifications
  • Active and unrestricted Nurse Practitioner license in the state of Texas.
  • Certified Nurse Practitioner through a national board: graduate of an accredited master’s degree in nursing (MSN) program and board certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC) in one of the following specialties – Adult‑Gerontology Acute Care NP, Adult/Family or Gerontology NP. Preferred certifications include ANP, FNP, or GNP.
  • Ability to move a 30‑pound bag in and out of a car and navigate stairs and varied dwelling conditions.
  • Availability to work 10 hours per week.
  • Reliable transportation enabling travel to client and/or patient sites within a designated area.
  • Proficient computer skills, including documentation in electronic medical records.
Preferred Qualifications
  • At least 1 year of nursing home experience.
  • Experience meeting the medical needs of patients with complex behavioral, social and/or functional needs.
  • Understanding of geriatrics and chronic illness.
  • Understanding of advanced illness and end‑of‑life discussions.
Compensation and Benefits

Compensation ranges from $90 to $155 per hour, inclusive of base pay and bonus. Additional benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase opportunities, and a 401(k) contribution. Eligibility requirements apply.
