Job Type: Remote, but required to come onsite 1 time a week

Summary

The Nurse Navigator collaborates with providers and or clinical areas to coordinate service and ensure quality of care for patients. The nurse independently assesses, evaluates and implements care delivered for a designated population of patients. The Nurse Navigator provides patients, families, and caregivers with tailored support along the care continuum to expedite and coordinate care and address health system barriers. With an emphasis on high‑risk patients or those with complicated care needs, the Nurse Navigator coordinates patient care through interdisciplinary and multidisciplinary teamwork to obtain optimal patient outcomes.

The Nurse Navigator is focused on enhancing care transitions and patient adherence. The Nurse Navigator helps establish and maintain ties with community resources to create a seamless model of care that benefits patients, family members, clinicians, and the healthcare team.

Responsibilities
  • Serves as primary contact and advocate for patient; builds and keeps trust of patient and family; provides emotional support; navigates the health care system using knowledge of the intricacies of each system and the patient advocate assuring timely access.
  • Provides education and information to the patient and family, helping to make care seamless, continuous, and comprehensive; initiates and documents patient teaching including family and significant others based on assessment of needs.
  • Responds to patient request for information regarding the disease process and community resources; uses appropriate patient education documentation modality.
  • Utilizes disease‑specific clinical pathways to ensure effective clinical disease management.
  • Collaborates with the health care team to identify resources available for the patient/family; coordinates the provision of education for patient and family regarding plan of care and health care needs; assures implementation and monitors pathways and patient care protocols.
  • Collaborates with health care team on plan of care, referrals and ongoing needs of the patient; participates in communication and coordination of this plan of care with the social worker and case manager and other health care team as necessary.
  • Improves quality through reduction in treatment delays, use of clinical pathways and monitoring of quality indicators.
  • Attends appropriate clinical and professional organizations, workshops and meetings.
  • Stays abreast of community resources available to facilitate safe patient transitions of care.
  • Remains current on clinical advancements related to specific disease process or diagnoses.
  • Participates in tracking and monitoring metrics and outcomes.
  • Participates in research as appropriate.
  • Demonstrates customer‑focused interpersonal skills, utilizing problem solving process and critical thinking.
  • Performs other duties as assigned.
How You’ll Be Successful Work Experience
  • At least 4 years clinical pediatric nursing experience Required.
  • At least 1 year case management; pediatric nursing experience in Oncology/Blood Disorder, Cardiology, NICU, ENT, GI, Neurology, ICU, Pulmonology, Nephrology preferred.
Education
  • Four‑year Bachelor’s degree or equivalent experience BSN Required.
Licenses and Certifications
  • Current license to practice professional nursing in the State of Texas required upon hire.
  • Basic Life Support for Healthcare Providers as required by CP 1.20 Life Support Course Requirements Required.
  • Accredited Case Manager (ACM), Certified Case Manager (CCM), Navigation Certification, or Certification in the appropriate specialized field within 4 years of hire into role Required.
