Oncology Nurse Navigator - RAPID Clinic

Position:
Full-Time;
Hybrid Role - 3 Days in Clinic, 2 Days Remote

Job Summary

The Oncology Nurse Navigator will support our new RAPID clinic. RAPID (Rapid Assessment for Potential Incidental or Diagnosed Malignancy). The Nurse Navigator calls patients within 24 hours, develops relationships, assesses for social determinants of health barriers, ensures records and documentation are available for the patient’s first visit, and navigates patients to diagnosis with hand‑offs to the care team.

This role assists patients, families, and caregivers in overcoming healthcare barriers (social, financial, physical). Responsible for ongoing needs assessments and facilitating referrals to support services throughout the oncology continuum of care. Uses the nursing process and evidence‑based knowledge to provide education and resources that facilitate informed decision making by the patient and/or family.

Experience and Education

Required

  • Education
    :
    Associate’s Degree graduate of an approved/accredited nursing program by a governmental, nursing board, or licensing authority that has jurisdiction over approval/accreditation of nursing programs (Texas BON 217.1)
  • Experience
    : 3 years oncology nursing experience
  • Licenses and Certifications
    :
    Registered Nurse (RN) and Basic Life Support (BLS) course accredited by the American Heart Association or American Red Cross

  • Licenses and Certifications
    :
    Oncology Certified Nurse (OCN), Certified Pediatric Hematology Oncology Nurse (CPHON), Certified Breast Care Nurse (CBCN), Blood Marrow Transplant Cert RN (BMTCN)
Job Duties
  • Assess for barriers to care throughout the cancer care continuum to address patient, caregiver, or family needs.
  • Serve as the point of contact for provider, patient, and caregiver.
  • Serve as a patient advocate to assist the patient and caregiver through the health care system.
  • Facilitate education of patient and staff by developing appropriate education programs, resources and tools to address needs.
  • Provide education, resources and referrals to internal and external resources based on assessment needs.
  • Facilitate shared decision‑making, ensuring the patient, family and caregiver are fully informed of treatment intent, treatment, and outcomes.
  • Promote care planning by encouraging patients to convey their wishes and goals of care. Work with patients to formulate questions to discuss with their health care provider.
  • Facilitate clear and concise communication among providers, departments and patients.
  • Coordinate multidisciplinary planning and facilitate care conferences as necessary.
  • Participate in outreach projects to help close patient care gaps.
  • Perform other duties as assigned.
