Qualifications
  • Associate degree preferred
  • Certified Hospice Nurse (CHPN) preferred
  • Three+ years of experience in hospice, home health, general medical surgery, oncology, or a patient population‑specific nursing field
  • Minimum of one year of supervisory experience
  • Valid CPR certification while employed in a clinical role (preferred)
  • Licensed driver capable of traveling to all business locations
  • Meet the regulations and requirements of the state(s) where services are provided
Responsibilities
  • Supervise patient and family care team members, collaborating with program site leadership
  • Manage scheduling, payroll, and other administrative duties
  • Ensure all initial assessments and required reassessments are completed by qualified team members within required time frames, including documentation submissions
  • Oversee and support the Interdisciplinary Group (IDG) in developing, implementing, and reviewing the interdisciplinary plan of care for each patient and family
  • Collaborate with site leadership to monitor, manage, and mentor staff in patient care activities, productivity, and documentation per related policies and protocols
  • Participate in and promote Back to Bedside efforts to reinforce Case Management skills, and knowledge and attitude per protocol for all disciplines
  • Support staff education efforts in collaboration with site leadership
  • Facilitate IDG meetings and provide ongoing IDG support such as triaging incoming calls, providing caller support and assistance, and performing quality calls
  • Participate in or lead quality improvement activities, including data collection and aggregation as appropriate
  • Reinforce and support all internal and external communications
