USRC is a leader in the dialysis industry and values a diverse workforce. The Home Therapy Registered Nurse is a federally and state‑licensed RN who provides training and ongoing support to patients using home dialysis modalities.
Responsibilities- Provide patient (and/or care partner) training in self‑care for Peritoneal Dialysis or Hemodialysis in accordance with federal and state regulations.
- Conduct home visits to assess the patient’s environment, initially and at least annually thereafter, and reevaluate more frequently when needed.
- Document all nursing services in the Electronic Medical Record, including training sessions, routine and non‑routine in‑person interactions, and phone conversations.
- Participate in infection control monitoring, implementation, and record‑keeping.
- Review and communicate emergency preparedness procedures with patients, covering actions such as emergency disconnection and contact protocols.
- Maintain appropriate clinic and patient supply inventory, place orders in a timely manner, and avoid fees or penalties.
- Track and analyze program and patient performance data, using critical thinking and root‑cause analysis to improve outcomes.
- Obtain and communicate routine and non‑routine laboratory results, including critical lab values, to the responsible physician or extender.
- Review patient medication profiles monthly and administer medications as ordered.
- Perform on‑call nursing services—nights and weekends—on a rotational basis as needed.
- Flexibly accommodate staffing locations and hours to meet program needs.
- For Home Hemodialysis, coordinate home evaluation with the technical services department to ensure that electrical, plumbing, and drainage requirements are met, and oversee required water sampling and culture reporting.
- Registered Nurse licensed in the state, meeting federal and state regulations for home dialysis program.
- Experience with home dialysis modalities preferred.
