JOB SUMMARY

The Quality Assurance LVN, under the direct supervision of the Quality Assurance Nurse Supervisor, implements the Quality Improvement Program and supportive activities. Review clinical charting for conformance/compliance to Medicare and Agency standards. Participate in quality assurance and improvement activities for the agency.

Education & Qualifications

Education

Graduate of an accredited Licensed Vocational Nurse program.

Experience

2 years of experience in home health care.

Licenses and Certifications

(LVN) LICENSED VOCATIONAL NURSE Current licensure as a Licensed Vocational Nurse by Board of Nurse Examiners for the State of Texas (or BNE Compact State). Upon Hire

Preferred
  • Experience 2 years of Home Health Quality Assurance experience
JOB DUTIES
  • Audits daily notes for proper documentation and ensures chart reviews are timely, accurate, and meet all data requirements under CMS guideline.
  • Monitors compliance with both internal standards and regulatory requirements.
  • Discusses any noted documentation discrepancies with QA RN Supervisor and Clinical Managers.
  • Regularly participates in case conference activities and assists with record reviews that conferences are conducted and documented in accordance with Agency policy.
  • Participates in Quality Improvement (QAPI) Program and assures appropriate corrective measures are identified and implemented.
  • Assists in tracking QAPI items (infections, hospitalizations, incident reports, etc.).
  • Performs other duties as assigned.
