Contract (13 Weeks) | Full-Time | Onsite | Days | Saturday – Monday, 7:00 AM – 7:00 PM

• Perform hospital-based case management functions including assessment, planning, and coordination of patient care
• Facilitate discharge planning and transitions of care to ensure appropriate level of care and resource utilization
• Collaborate with interdisciplinary care teams including physicians, social workers, and ancillary staff to coordinate patient services
• Monitor patient progress and adjust care plans to meet clinical and discharge goals
• Ensure documentation of case management activities is accurate, timely, and compliant with facility standards

• Active Registered Nurse (RN) license valid in the state of Texas or compact licensure eligibility
• Minimum of 1 year of clinical experience in case management or a related hospital setting
• Strong knowledge of discharge planning, utilization review, and care coordination principles
• Ability to work a weekend-based schedule (Saturday through Monday, 12-hour day shifts)

• Prior travel nursing experience in a hospital case management role
• Familiarity with acute care utilization management and payer requirements
• Case Management certification (CCM or ACM) is a plus
