We are seeking an experienced and compassionate Hospice PRN Case Manager who is dedicated to providing exceptional end‑of‑life care with dignity, empathy, and clinical excellence, and who is willing to travel in DFW Area.

This role is ideal for a nurse who is passionate about providing high‑quality, patient‑centered end‑of‑life care while enjoying flexibility in scheduling. You will work closely with patients, families, and an interdisciplinary team to ensure comfort, dignity, and support throughout the hospice journey.

Service Areas:
  • Dallas Metro Area
Key Responsibilities:
  • Conduct comprehensive patient assessments and develop individualized care plans
  • Provide skilled nursing care and symptom management
  • Coordinate care with physicians, social workers, chaplains, and other team members
  • Educate patients and families on disease progression, medications, and care techniques
  • Monitor patient condition and adjust care plans as needed
  • Maintain accurate and timely clinical documentation
  • Participate in on‑call rotation as needed
  • Ensure compliance with hospice regulations and standards
Qualifications:
  • Current and unrestricted RN license in the state of Texas
  • Minimum 1–2 years of hospice, home health, or related experience preferred
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and interpersonal abilities
  • Ability to work independently and manage time effectively
  • Reliable transportation and valid driver’s license
  • Current CPR certification (required)

The salary is Negotiable Compensation: $90,000 Yearly
