Nurse Navigator

Methodist Health System is seeking a Nurse Navigator to join our Healthcare & Medical Services team. In this role, you will guide patients and families through the care continuum, coordinate appointments and services, and collaborate with physicians and clinical teams to ensure seamless, compassionate care. You will provide education, advocate for patients, and support complex care transitions. Responsibilities

• Serve as a central point of contact and advocate for patients and families throughout the care continuum.
• Conduct focused clinical and psychosocial assessments to identify patient needs, barriers, and risk factors.
• Develop, implement, and continually update individualized care and navigation plans in collaboration with the interdisciplinary team.
• Coordinate and schedule appointments, diagnostic tests, treatments, and follow-up visits across hospital and clinic settings.
• Educate patients and families about diagnoses, treatment options, procedures, medications, and self-care strategies.
• Promote adherence to evidence-based guidelines and clinical pathways to support quality, safety, and optimal outcomes.
• Facilitate smooth transitions of care between inpatient, outpatient, and community settings, including discharge planning and referrals.
• Document all patient encounters, education, and care coordination activities in the electronic health record accurately and timely.
• Participate in multidisciplinary rounds, case conferences, and performance improvement initiatives related to navigation and care coordination.
• Collaborate with physicians, advanced practice providers, case managers, social workers, and ancillary staff to address clinical and psychosocial needs.
• Identify and connect patients with financial, social, and community resources to reduce barriers to care.
• Monitor patient progress, track key metrics, and escalate clinical concerns promptly to the appropriate provider.
• Maintain compliance with organizational policies, regulatory standards, and professional nursing practice guidelines.
