Role Overview:
The Inpatient Registered Nurse (RN) Care Manager provides comprehensive, patient-centered care management for at-risk patients in an acute care setting.
Key Responsibilities:Conduct assessments and care planning, coordinate care, manage resource utilization, review and discharge planning, document interventions, ensure regulatory compliance, and advocate for patients. The goal is to ensure appropriate healthcare resource use across the continuum, delivering the right care at the right time and setting.
Qualifications & Skills:Requires an Associate's Degree in Nursing, Alaska RN license, 2 years of acute care experience, and competency in Utilization Review. Preferred qualifications include a Bachelor's Degree, national certification, and experience in care management or utilization review.
