Position Overview

Heal Bridge is seeking an experienced and compassionate Nurse Practitioner (NP) with a strong background in wound care to provide mobile wound management services across Skilled Nursing Facilities (SNFs) within the Dallas area.

This is a mobile, field-based role ideal for an autonomous and motivated clinician who thrives in a startup environment, enjoys flexible scheduling, and is passionate about delivering exceptional patient care.

Key Responsibilities
  • Conduct comprehensive wound assessments, develop individualized treatment plans, and perform follow-up evaluations.
  • Perform sharp/surgical, enzymatic, autolytic, and mechanical debridement as clinically indicated.
  • Utilize MIST Therapy (low-frequency, non-contact ultrasound) and other advanced wound modalities.
  • Obtain PCR testing samples for rapid wound pathogen identification and ensure appropriate documentation.
  • Administer or apply skin grafts, tissue substitutes, and other regenerative therapies to promote wound healing.
  • Collaborate with SNF nursing teams, physicians, and care coordinators to ensure continuity of care.
  • Maintain accurate and timely electronic charting in alignment with Heal Bridge clinical protocols and documentation standards.
  • Educate facility staff on wound prevention, dressing protocols, and infection control practices.
  • Uphold the highest standards of compliance, professionalism, and patient confidentiality.
Requirements
  • Education & Licensure: Active, unrestricted Nurse Practitioner license in the State of Texas, in good standing.
  • Experience: Minimum 1–2 years practicing as an NP, with demonstrated specialization or significant exposure in wound care.
  • Clinical Practice: Must have provided direct patient care within the past 6–12 months.
  • Credentials: Valid and current certifications, credentials, and licenses required for practice in Texas.
  • Availability: Willing and able to work approximately 40 hours per week within Heal Bridge’s standard operating hours.
  • Transportation: Reliable transportation and valid driver’s license for travel between assigned SNFs.
