Join our Love Well team as a Full-Time RN Case Manager serving patients and families throughout the Dallas area. In this role, you'll provide expert clinical care, compassionate support, and guidance to patients and their loved ones during some of life's most meaningful moments.

As an RN Case Manager, you will coordinate and oversee patient care, collaborate with the interdisciplinary team, and serve as a trusted resource for patients and families. The ideal candidate combines strong clinical skills with emotional intelligence and a genuine passion for hospice care.

Qualifications- Active Texas RN license
- Active CPR
- 2 years of experience with a Hospice agency preferred.
- Fire Note experience preferred.
- Good communication skills, assessment skills, and strong clinical documentation skills.
- Ability to read, write, and speak the English language.
- Reliable transportation, valid drivers license and current auto insurance.
