Description:

This position involves working Monday through Thursday. The shift is from 5:00 PM to 3:00 AM with a 1-hour lunch break. On-call duty is from 3:00 AM to 8:00 AM each night, shared between two RNs. You may or may not receive triage calls or make visits during on-call hours. Experience in outpatient hospice is mandatory. The role involves providing end-of-life care to patients, primarily adults, with some pediatric cases.

This position includes hospice referrals and admissions. Skills required include pain assessment, medication tolerance, and patient/family education. Experience with clinical documentation of hospice eligibility is preferred. Candidates must understand the analgesic ladder and non-pharmacological pain interventions. Proper disposal of medications and effective communication with families is essential. You will be working collaboratively with interdisciplinary support including social services and pharmacy.

Individuals must be ready to complete needed lab items within a specified timeframe upon accepting the position.
