Field- RN Case Manager: Wound Care & Infusion

Adaptive Home Health is seeking an experienced RN Case Manager to deliver high-acuity home health care in the field across Texas. You will own patient assessments, document visits including OASIS, and coordinate with physicians and families to keep plans of care on track.

This full-time role offers a field-based schedule with productivity-based compensation and a supportive clinical team to help you lead LVNs and HHAs while improving patient outcomes.
