YOUR CONTRIBUTION TO OUR SUCCESS!A Housing Care Manager - DHA contributes to AHF by supporting clients referred by Housing Forward to access and maintain stable housing placement in The Village Project-Based Permanent Supportive Housing Program. The Housing Care Manager - DHA works closely with potential and current residents to identify barriers to housing, develop individualized care plans, and facilitate access to resources that promote long-term housing stability.
Through ongoing engagement and follow-up, they ensure clients receive the necessary services to remain housed. The Housing Care Manager – DHA collaborates with Housing Forward and the Dallas Housing Authority (DHA) to maintain alignment with continuum standards of care.
A Housing Care Manager - DHA enhances the reputation of AHF by providing comprehensive, onsite support to residents of the 28 project-based voucher units embedded within The Village of Dallas property. The Housing Care Manager - DHA facilitates the housing process by accepting referrals through Housing Forward, assisting clients in the DHA application process, and coordinating housing placement with Village property management.
Once housed, they deliver ongoing care management focused on housing retention, including a minimum of two in-home visits per client each month, needs assessments, care plan development, and timely referrals as needs arise. Acting as a liaison between tenants and property management, the Housing Care Manager - DHA ensures clients understand lease terms, addresses concerns, and communicates with DHA regarding lease violations or potential exits.
In cases of program termination, they support clients through the transition, notify Housing Forward and DHA, and help connect them to alternative housing options.

The Housing Care Manager - DHA will work as a part of a close-knit team to support residents of The Village of Dallas by collaborating with Village staff, Housing Forward, DHA, and other community partners. They will become familiar with both DHA’s and Housing Forward’s intake, recertification, and exit processes to ensure smooth transitions into housing and continuity of care. This position requires travel to engage clients where they are at the point of referral.
