- Licensed Social Worker, active license in the state of practice.
- Bachelor’s Degree in Social Work or a related field (Sociology, Special Education, Rehabilitation Counseling, Psychology) from an accredited school of social work.
- One year of supervised social work experience in a healthcare setting working directly with geriatric individuals.
- Minimum of 2 years of nursing experience in a Skilled Nursing Facility (preferred).
The Social Worker is responsible for identifying psychosocial, mental and emotional needs and providing, developing, and aiding in the access of services to meet those needs. Services are delivered in accordance with the National Association of Social Workers (NASW) Code of Ethics and federal, state, and local regulations.
Responsibilities- Complete social history and psychosocial assessments for each resident, and complete relevant parts of the Minimum Data Set (MDS) to guide care planning.
- Maintain accurate documentation that complies with federal/state regulations and corporate policies, including Advanced Directives, Social Service History and Assessment, Raps, Care Plans, Social Service Progress Notes, Behavior Tracking, and Discharge Planning.
- Support residents’ right to self‑determination and document refusals of services, informing residents of alternatives and involving others as appropriate.
- Develop interdisciplinary care plans that address psychosocial needs, goals, and appropriate interventions.
- Provide therapeutic interventions to help residents adjust to long‑term care, addressing social, emotional, and psychological needs.
- Offer support and education to residents, families, and significant others, and refer them to external social service agencies when needed.
- Coordinate discharge planning and refer for home‑care services before residents return to the community.
- Collaborate with the interdisciplinary team and administration to promote resident rights and well‑being, preventing abuse as mandated by law and professional licensure.
- Assist residents, families, staffing and community members in adopting self‑advocacy and improving quality of life and care.
- Help plan, organize, implement, and evaluate the facility’s social service programs under the Social Service Director/Consultant.
- Prepare for inspection surveys, engage with inspectors, implement corrections, and reinforce standards.
