Overview

This position is responsible for interviewing, accepting, and providing comprehensive, structured, complex case management services for an assigned caseload of clients participating in life management programs such as domestic violence outreach, Pathway of Hope, and Prevention services. The case manager understands the uniqueness of each client’s history to determine the most effective program plans, develops comprehensive program goals, evaluates client progress through mentoring and counseling, and serves as an advocate to acquire services that enable clients to cope functionally with their environment.

The role ensures constant compliance with funding requirements and researches and obtains funding for the Pathway of Hope program.

Key Responsibilities
  • Interview applicants to determine eligibility for program enrollment based on established facility guidelines and requirements; conduct needs assessment and obtain pertinent information; refer applicants to other agencies if not appropriate for the program.
  • Establish unique comprehensive program goals (three months or more) for eligible clients based on personality, decision‑making abilities, mental capabilities, addiction history, family involvement, etc.; explain goals to client in a manner that is easily understood; consistently counsel client on ways to modify or stop negative behaviors while in the program.
  • Outsource clients to additional counseling resources if needed and maintain awareness of progress with external case‑management professionals, working as a team to meet the holistic needs of the individual.
  • Provide direct assistance in obtaining and maintaining self‑sustaining sources of income, benefits, and other economic supports as well as professional resources that enhance clients’ psychosocial well‑being.
  • Meet regularly with clients to discuss and evaluate their progress, feelings, impressions, changes, and personal growth/development; prepare accurate and up‑to‑date records documenting the same; receive incident reports and enforce disciplinary actions for infractions of the house guidelines.
  • Facilitate individual and group discussion to ensure clients’ attitudes and motivations are progressing; redirect negativity, promote self‑awareness and provide appropriate encouragement.
  • Plan, coordinate, and/or facilitate life‑management classes to assist clients in obtaining skills that will enable them to functionally cope with their environment; obtain speakers and workshop presenters as needed.
  • Maintain awareness of the program requirements necessary to maintain existing funding; ensure that measurable outcomes correspond with requirements and make adjustments to case‑management processes as necessary.
  • Analyze statistical information and case outcomes to ensure that recidivism is not occurring on a regular basis; recommend and implement changes to program guidelines to reduce repetitive requests for service.
  • Assist clients in locating and securing affordable housing; work closely with client to transition into the new housing; conduct home visitations when appropriate; ensure that client’s living quarters are properly furnished and maintained; contact appropriate person to facilitate needed repairs; conduct routine inspections of living quarters and grounds when applicable.
  • Refer clients to appropriate Corps programs or other agencies based on clients’ needs and in accordance with their program plan.
  • Complete and articulate discharge plans for clients exiting the program.
  • Prepare and maintain case records and logs on all assigned clients; ensure the accuracy and completeness of the same; enter pertinent information into the established Homeless Management Information System (HMIS).
  • Attend community meetings with representatives of other agencies to educate them about the program, make client referrals and maintain awareness of client’s progress.
  • Prepare and maintain statistical records on all services provided; comply with and prepare monthly statistical reports; ensure the accuracy and completeness of the same.
