Independent Contractor-Family Support Services

Health Connect America, Inc. seeks an Independent Contractor - Family Support Services professional to provide in home and community-based support to children, adolescents, and families. In this role, you will implement family-centered service plans, coach caregivers on behavior and skill-building strategies, provide parenting education, link clients to community resources, and collaborate with clinicians, schools, and agencies. You'll document visits, monitor progress, follow safety and confidentiality standards, and maintain timely records while supporting client well being in partnership with a compassionate, mission driven care team.

Responsibilities

• Provide in-home and community-based family support services to children, adolescents, and caregivers.
• Implement and monitor individualized service or support plans.
• Coach parents/caregivers in behavior management, skill-building, and parenting strategies.
• Connect families to community resources and coordinate with schools, agencies, and providers.
• Document visits, progress notes, and service activities accurately and on time.
• Monitor family safety and risk, following crisis and escalation protocols as needed.
• Collaborate with clinicians and multidisciplinary teams to align support with treatment goals.
• Maintain professional boundaries, confidentiality, and ethical standards.
• Participate in training, supervision, and team meetings as required.
• Travel regularly within the community to provide services in homes and community settings.
