Weekend Admissions Coordinator

Are you organized, compassionate, and great at building relationships? Join our skilled nursing facility team as an Admissions Coordinator and play a key role in welcoming new residents and supporting families through the admission process.

The Admissions Coordinator is responsible for managing the referral and admissions process, maintaining census goals, and serving as a liaison between the facility, hospitals, families, and the community each weekend.

• Manage incoming referrals and coordinate the admissions process from start to finish
• Communicate with hospitals, case managers, physicians, and families
• Verify insurance coverage and obtain necessary authorizations
• Conduct facility tours for prospective residents and families
• Maintain accurate admission records and documentation
• Collaborate with clinical and administrative teams to ensure smooth transitions
• Support census development and marketing initiatives

• Previous experience in admissions, healthcare, or SNF preferred
• Knowledge of Medicare, Medicaid, and managed care processes
• Strong communication and interpersonal skills
• Excellent organizational and multitasking abilities
• Proficient in basic computer systems and EMR platforms
• Ability to work in a fast-paced environment
