Job Overview

The Center of Love and Hope is seeking a compassionate and reliable Home & Community Support Specialist to assist individuals in achieving greater independence within their homes and communities. This role focuses on providing person-centered support, promoting skill-building, ensuring safety, and encouraging meaningful daily routines.

The ideal candidate loves working with people and has experience supporting individuals with intellectual and developmental disabilities and is committed to fostering dignity, independence, and a high quality of life.

  • Provide support with Activities of Daily Living (ADLs) such as grooming, hygiene, meal prep, and household tasks
  • Support individuals in developing daily living skills, community navigation, and independence
  • Assist with meal preparation, ensuring safe food handling and supporting healthy choices
  • Offer companionship, social engagement, and encouragement to participate in meaningful activities
  • Maintain a clean, safe, and organized environment (light housekeeping, sanitation, etc.)
  • Follow individualized support plans, behavior plans, and safety protocols
  • Document daily notes, progress, and observations accurately in compliance with agency and DDS standards
  • Support individuals during community outings, appointments, or errands
  • Respond appropriately to emergencies and follow agency procedures
  • Communicate effectively with team members, supervisors, and the individual’s support network
  • Promote dignity, respect, and person-centered care at all times
  • Experience in home care, community support, group homes, or similar environments
  • Familiarity with supporting individuals with intellectual and developmental disabilities
  • Ability to assist with ADLs and perform light housekeeping tasks
  • Strong communication skills and the ability to build trust and rapport
  • CPR and First Aid certification preferred (or willingness to obtain)
  • Ability to maintain confidentiality and follow HIPAA guidelines
  • Strong problem-solving skills, patience, and compassion
  • Ability to work independently and as part of a cohesive team
  • Must be able to lift, assist with mobility, and support individuals safely during transfers (as needed)
  • Reliable, responsible, and committed to providing high-quality support
