SummaryThe Patient Concierge Associate is responsible for managing the Discharge Lounge while welcoming and greeting patients and meeting the patient’s needs as they wait to be discharged from the hospital. This role will manage the patient experience while in the lounge and ensure a smooth transition from the hospital.
Responsibilities- Meet and greet patients in a professional and courteous way.
- Assure patients are comfortable and their needs are supported.
- Ensure the lounge area is maintained and presented to the highest possible standard.
- Respond quickly to patient requests and manage the patient experience.
- Log and maintain accurate records of each patient visit.
- Monitor and assist discharged patients and ensure discharge procedures are being followed.
- Round on patients in units to assure excellent customer service and increase loyalty.
- Provide feedback to the Director on patient issues that need to be addressed.
- Maintain and model Nuvance Health values.
- Demonstrate regular, reliable, and predictable attendance.
- High school graduate or equivalent.
- Team player with the ability to work in collaboration with all levels of employees.
- Flexible and able to adapt to change in a team‑based environment.
- Effective oral, written, and verbal communication and customer service skills.
- Proficient computer skills including Microsoft Word and Excel.
- Proficient EMR skills.
Manual:
Some manual skills/motor coordination & finger dexterity.
Occupational:
Little or no potential for occupational risk.
Physical Effort:
Medium to Heavy effort; may exert up to 35 lbs. force.
