Connecticut Institute For Communities, Inc. (CIFC) / CIFC Health seeks to hire a full-time Peer Recovery Coach to work in our Behavioral Health Department. Under the direction of the Lead BH Community Health Worker, this position is responsible for providing support and guidance to individuals seeking substance use disorder treatment. The peer recovery coach, an individual in sustained recovery, can utilize their experiences to mentor or coach, provide and advocate effective recovery‑based services, and offer peer mentoring inclusive of social skills, substance use recovery supports, training, mentoring, behavioral skills management, employment, and recreational activities.

The peer recovery coach will assist the team in supporting clients’ goals for recovery and providing interventions to help clients navigate their recovery journey. The position also includes providing supportive social services, bi‑directional referrals, follow‑up as required under CIFC Health’s standard of care, and assisting with financial and insurance support for CIFC Health patients and families seeking behavioral health/substance use disorder treatments.

Additionally, the coach supports CIFC Health providers by assisting with substance abuse treatment support and referrals.

• Provide education on substance use disorder and skills training in illness and recovery management to clients and their families.
• Work collaboratively with clients to assess their hopes, strengths, accomplishments, and challenges in regards to their recovery goals.
• Collaborate with other care team members to develop and assess client needs, creating goals and treatment plans that include personal interests, strengths, and challenges related to recovery.
• Identify and support referral and linkages to community resources (recoveries, educational, vocational, social, cultural, spiritual resources, mutual self‑help groups, professional services, etc.) that support client recovery goals and interests.
• Provide in‑person and telephonic support, coaching, and mentoring to help clients meet their recovery goals.
• Provide informal counseling, discuss issues and concerns related to basic needs and community resources, and facilitate peer recovery groups.
• Facilitate navigation of healthcare and social service systems.
• Screen for social determinants of health.
• Determine eligibility and enroll individuals into health insurance plans.
• Act as point person receiving and facilitating referrals for Substance Use Disorder treatment services, collaborating with providers internally and externally, including referrals to withdrawal management and inpatient substance abuse/co‑occurring treatment programs.
• Collect clinical data and records prior to intake appointments, including records from other facilities, and administer rating packages completed by patients and collateral as appropriate.
• Communicate with other providers regarding substance use disorder referral outcomes by creating provider letters, discharge letters, and summaries in cooperation with providers.
• Participate in clinical team meetings and communicate with providers involved in patients’ care to facilitate coordination.
• Attend provider team meetings to promote service collaboration.
• Participate in individual and/or peer supervision as directed.
• Adhere to all HIPAA regulations, including heightened protection of health records, and maintain confidentiality at all times.
• Fulfill all compliance and training responsibilities related to the position, including enforcement of CIFC and CIFC Health policies and procedures.
• Perform other related duties as assigned.

Successful candidates must possess a minimum High School Diploma or equivalent (Associate/Bachelor’s Degree preferred), lived experience and/or be in recovery from substance use for a minimum of 2 years, and either hold or be willing to obtain Certified Addictions Recovery Coach status. Community health experience and/or community resource knowledge is strongly preferred. Computer and electronic health record experience, including internet use, is required.

Bilingual abilities are preferred.

Hourly Wage: $20‑$24
