Trinity Health Of New England At Home is seeking a Home Care RN to provide primary nursing care in patients' homes within the Hartford Region. A $10,000 sign-on bonus is offered and paid over two years.
Responsibilities include initial assessments, care planning, therapy delivery, and care coordination with the Population Health team. Schedule is Monday–Friday, with one weekend every four weeks and two rotating holidays per year.
