Summary:

Provides professional care to patients utilizing the nursing process in collaboration with other healthcare team members. Works on a per diem basis.

Responsibilities:

• Provides assessment of physical, emotional, socio-economic, and environmental needs of patients and their families.
• Identifies problems utilizing patient database, nursing process, and physician's plan of treatment.
• Develops a care plan and explains diagnosis and treatment implications to patients and families.
• Observes, evaluates, and reports patient conditions to the physician, including reactions to treatments.
• Assesses health status through observation and interviews; counsels families on wellness principles and related fields.
• Performs screening and diagnostic procedures; assists physicians during examinations and treatments.
• Interprets test findings, makes referrals, and obtains laboratory specimens.
• Attends continuing education and participates in Quality Management programs.
• Fulfills all compliance responsibilities and performs other duties as assigned.

Required:

Current RN License in Connecticut

Minimum Experience:

0-6 months
Desired: BSN and experience in a qualified home health agency.

Working Conditions:

Manual skills, occupational risks, exposure to dirt, odors, noise, and human waste.
