- Assess physical, emotional, socio‑economic, and environmental needs of patients and families.
- Provide services ordered by the physician as indicated in the plan of care. Communicate with all physicians involved and other health‑care practitioners as appropriate.
- Develop, implement, and evaluate the plan of care in partnership with the patient, representatives, caregivers, and physicians.
- Coordinate services with the patient, family, and others involved in the care plan. Provide patient, caregiver, and family counseling and education.
- Regularly evaluate patient progress, take prompt action when changes in condition are noted or reported, and terminate care when goals are attained or agency policy requires termination.
- Participate in the agency quality assurance and performance improvement program, and attend agency‑sponsored in‑service training.
- Identify patient and family needs for other home health services and refer appropriately.
- Supervise services rendered by homemaker/home health aide in accordance with state, federal, Joint Commission standards, and submit periodic evaluations to the Home Health Aide Supervisor.
- Fulfill all compliance responsibilities, including preparing clinical notes and attending in‑service training.
- RN diploma with over 3 years of job‑related experience considered in lieu of an Associate’s Degree.
- Current RN license in the state of Connecticut.
- Strong research skills, clear and concise interpersonal communication, and knowledge of systems integration.
- Minimum one year of experience; desired experience in a qualified home health agency.
- Knowledge of operating in a home health environment (manual skills, motor coordination, finger dexterity).

Standard hours: 40 per week
Schedule:
Day 8, Monday‑Friday 8:30 AM‑5 PM with weekend rotation
Salary range: $41.00‑$67.05 per hour
