Aortic Disease Program Coordinator- NP/PA

Summary:

The Aortic Disease Program Coordinator is a clinical and operational leader responsible for supporting the development, implementation, and growth of a multidisciplinary Aortic Disease Program across cardiac surgery and vascular surgery. This role focuses on clinical coordination, operational execution, and program integration across inpatient and ambulatory settings. The coordinator works closely with physician leadership, institute administration, and system teams to enhance patient care, streamline workflows, and support program growth in a clinically and operationally matrixed environment.

The coordinator will play a key role in complex care coordination, patient navigation, and longitudinal disease management, while contributing to quality outcomes, patient experience, and program development.

Responsibilities:
1. Clinical Program Coordination
  • Facilitates coordination of multidisciplinary care for patients with aortic pathology
  • Ensures appropriate diagnostic workup, imaging, and pre-procedural readiness
  • Assists with longitudinal surveillance and follow-up pathways for aortic patients
  • Supports case preparation and multidisciplinary case conferences in collaboration with physician leadership
2. Operational & Workflow Management
  • Facilitates development and optimizes workflows for referral intake, diagnostics, treatment, and follow-up
  • Coordinates complex case logistics (OR scheduling, vendor/device coordination, imaging readiness)
  • Partners with administrative leadership on access, throughput, and care coordination efficiency
  • Supports consistency of care pathways across multiple sites
3. Care Coordination & Patient Experience
  • Oversees navigation processes to ensure seamless patient transitions across care settings
  • Serves as a key clinical resource for patients and referring providers
  • Integrates genetic counseling and family screening into care pathways where appropriate
  • Ensures a high-quality, patient-centered experience throughout the care continuum
4. Quality & Performance Improvement
  • Supports development and tracking of program metrics (access, outcomes, follow-up compliance)
  • Collaborates with Quality and Data Analytics teams to support reporting and process improvement
  • Participates in quality initiatives aligned with institutional and national benchmarks
5. Program Development & Growth
  • Supports physician and administrative leadership in program growth initiatives
  • Assists with outreach coordination and referral pathway development
  • Contributes to development of new programmatic components (e.g., surveillance clinics, genetic programs)
6. Research & Innovation
  • Supports clinical research efforts, registries, and data collection
  • Assists with coordination of clinical trials and IDE-related workflows
  • Helps integrate emerging technologies and best practices into program operations
7. Team Collaboration
  • Works in close collaboration with physician leadership, nursing, administration, and ancillary services
  • Supports a culture of collaboration, accountability, and continuous improvement
  • Assists in onboarding and supporting team members involved in the program
Education
  • Master's degree required (Nurse Practitioner or Physician Assistant Studies)
Licensure/Certification
  • Current CT license as Nurse Practitioner or Physician Assistant
  • Board Certification required
  • ACLS/BLS required
Experience
  • 5+ years of cardiovascular, vascular, or surgical clinical experience preferred
  • Experience in complex care coordination or multidisciplinary care environments
  • Demonstrated ability to work across teams and manage clinical workflows
Preferred
  • Experience in aortic disease, cardiac surgery, vascular surgery, or advanced cardiology
  • Experience with clinical programs, quality initiatives, or registries
  • Exposure to genetic or familial disease programs
  • Interest in program development and innovation

Salary: $115k-$160k (commensurate with experience and internal equity)
