About This Role
The Registered Nurse (RN) in Med/Tele, PCU, and ICU Contracts at Northwell Health is responsible for providing comprehensive patient care through assessment, planning, implementation, and evaluation. - Perform an age-specific plan of care for a designated group of patients.
- Collaborate with physicians and health team members to implement procedures and treatments.
- Use leadership skills to coordinate patient care and delegate activities.
- Assess the patient's physiologic health status.
- Conduct patient interviews and explain policies and procedures.
- Assess gastrointestinal, cardiovascular, respiratory, renal, and neurological health status.
- Determine mobility, sensory deficits, and skin condition.
- Assess level of pain and pain management.
- Communicate and document patient's health status and plan of care.
- Assess patient's psychosocial health status.
- Elicit perception of medical/nursing care and expectations.
- Determine coping mechanisms and knowledge level.
- Identify cultural/ethnic requirements to reduce anxiety.
- Communicate and document psychological status and care plan.
- Identify patient outcomes.
- Develop criteria for measurement of patient outcomes.
- Identify actual/potential patient problems.
- Identify patient's need for teaching based on assessments.
- Develop individualized patient goals.
- Formulate the plan of care and establish priorities.
- Identify care activities and establish priorities.
- Coordinate cost-effective use of supplies and medications.
- Document the plan of care and collaborate with health team members.
- Implement the plan of care.
- Maintain constant vigilance over patient care and safety.
- Reassess patients and revise plan of care as needed.
- Administer prescribed treatments and document accurately.
- Provide emotional support to patients and families.
- Evaluate care provided for patient outcomes.
- Measure effectiveness of care and document findings.
- Perform variance analysis for performance improvement.
- Design and evaluate systems to improve care in the unit.
- Collaborate with care team members in treatment planning.
- Collaborate with physicians on patient's plan of care.
- Assist physicians during treatments and diagnostic procedures.
- Use clinical judgment in delegating assignments to care team members.
- Administer medications correctly and safely.
- Identify medication actions, dosages, and side effects.
- Administer and document medications accurately.
- Participate in patient and family education.
- Provide explanations of conditions and treatment plans.
- Evaluate effectiveness of teaching through feedback.
Education: Associate’s Degree in Nursing required, Bachelor’s Degree in Nursing preferred.
Experience: Minimum 1–2 years of professional nursing experience in the related specialty.
Licenses &
Certifications:
Current RN License in New York State and/or Connecticut required. BLS certification required, ACLS preferred.
