Summary:

The Program Coordinator is a resource and educator for physicians, nurses, and other clinicians dealing with ostomy, wound, skin and incontinence issues. Provides consultative work and bedside education on patient care units in regards to skin management. Participates in initiatives to improve performance related to care. Supports nursing practice in other quality initiatives.

Required:

Bachelor's Degree required. State of CT RN license required. BLS certification. Expert clinical, critical thinking and patient care management and nursing. Expert in ostomy, wound, skin and incontinent patient care.

Minimum Experience:

three years

Desired:
Certification in Wound, Ostomy, Continence care. Performance improvement; team leadership skills.

Shift: Monday-Friday, 8:00am-4:30pm

Responsibilities:

  • Provides clinical leadership and expertise in area of specialty. Assures that policies and procedures related to ostomy, wound, skin and incontinence patients are implemented through collaboration with physicians, nurses and the interdisciplinary team.
  • Develops, plans, coordinates, and implements formal training initiatives on wound, ostomy, continence educational programs applying the principles of adult learning and education practice in accordance with the mission and philosophy of the Department of Nursing and Danbury Hospitals.
  • Coaches and educates staff to include physicians, nurses and other clinicians dealing with ostomy, wound, skin and incontinence issues. Revises program as needed.
  • Performs assessment and coordinates management of wound, ostomy, skin and continence services.
  • Assures nursing interventions follow national and Danbury Hospitals standards of nursing practice for treatment of wound, ostomy, skin and/or continence problems, providing recommendations for management, education to staff and to patients, enhancing patients' self-care abilities, and working toward positive patient outcomes. Works collaboratively with physicians in developing protocols.
  • Provides counseling and correction of practice habits of nursing staff.
  • Analyzes data specific to wound, ostomy, skin and incontinence patients regarding trends and outcomes to monitor and promote process improvements in their care to include but not limited to collection, tracking, and trending of pressure ulcer incidence and prevalence.
