Licensed Practical Nurse - Skilled Nursing & Rehab

Care Rite Centers is seeking a compassionate LPN to deliver resident-centered nursing care in our skilled nursing and rehabilitation centers. In this full-time, on-site role, you will administer medications and treatments, monitor vital signs, support rehabilitation plans, and document in the EHR. You will collaborate closely with RNs, therapists, and physicians to enhance resident comfort, safety, and outcomes. Responsibilities

• Administer prescribed medications and treatments in accordance with state regulations and facility policies
• Monitor and document residents' vital signs, symptoms, and responses to care in the EHRAssist with admission, transfer, and discharge processes, including assessments and documentation
• Support RNs and interdisciplinary team in developing and implementing individualized care plans
• Provide direct bedside care, including hygiene, mobility assistance, and comfort measures
• Communicate changes in resident condition promptly to RNs, physicians, and families
• Maintain infection control, safety, and fall prevention standards throughout the unit
• Participate in care conferences, quality improvement initiatives, and in-service trainings
• Educate residents and families on care plans, medications, and health management
