Weekend LPN

Hartford Healthcare is seeking a compassionate Weekend LPN to join our Healthcare & Medical Services team. In this role, you will provide direct patient care, administer medications and treatments, monitor and document patient status, and collaborate with RNs, physicians, and interdisciplinary teams. You'll educate patients and families, support care plans, and maintain safety and infection control standards. Responsibilities

• Provide direct bedside nursing care to patients on weekend shifts
• Administer medications and treatments per provider orders and facility protocols
• Monitor, document, and report changes in patient condition using EMR systems
• Assist with assessments and contribute to individualized care plans
• Perform wound care, vital signs, and basic diagnostic support tasks
• Educate patients and families on treatments, medications, and discharge instructions
• Maintain infection prevention and safety standards
• Collaborate with RNs, physicians, and the interdisciplinary care team
• Support admissions, transfers, and discharges on the unit
• Accurately complete documentation and handoffs between shifts
