About This Role

The Registered Nurse (RN) in Home Care at Northwell Health is responsible for providing comprehensive patient care through assessment, planning, implementation, and evaluation. This role is ideal for nurses who are skilled in collaborating with healthcare teams and managing patient care effectively.

• Location: Danbury, CT
• Schedule: Full-Time

• Assess the patient’s physiologic health status.
• Conduct patient interviews and explain policies and procedures.
• Assess gastrointestinal, cardiovascular, respiratory, renal, and neurological health status.
• Determine mobility, sensory deficits, prostheses use, and skin condition.
• Assess level of pain and pain management.
• Communicate and document patient’s physiologic health status and plan of care.
• Assess patient’s psychosocial health status.
• Elicit perception of medical/nursing care and expectations.
• Develop criteria for measurement of patient outcomes.
• Formulate the plan of care and establish priorities to achieve expected outcomes.

• Education: Associate’s Degree in Nursing required, Bachelor’s Degree in Nursing preferred.
• Experience: Minimum 1-2 years of professional nursing experience in the related specialty.

  Certifications:
  
  Current license to practice as a Registered Professional Nurse in New York State and/or Connecticut required. BLS certification required, plus specialized certifications as needed.
