Title: Residential Register Case Manager
Job Summary: Supervises/assists with the coordination of all aspects of health care for the individuals served within our residential homes in collaboration with the treatment team. Ensures individuals served achieve their maximum level of health and wellness. Supervises overall clinical operations of the residential home.
SCHEDULE: Monday through Friday 9:00 AM-5:00 PM
Pay: $48.00 - $49.50 hr.
- RN Case Manager is responsible for communicating and collaborating with Program Directors to obtain and review medical records, current medications, health status, and functional abilities consistent with ISP and Annual Physical
- Hires, trains, schedules, and supervises LPNs for Rowley Bridge Road.
- Ensures staff/LPNs receive required medical and clinical training; serves as a MAP coordinator.
- Identifies learning needs and develops or secures appropriate training for staff at varying levels of licensure, including but not limited to RN, LPN, direct care staff, program directors, and assistant program directors.
- Responsible for acting as an advocate and liaison for the individual to ensure that they are receiving a level of care consistent with clinical best practices.
- Conducting a comprehensive nursing assessment of the individual’s health and psychosocial needs and developing a Case Management Plan collaboratively with the individual, family and caregivers.
- Complete DDS Health Care Record and Health and Dental within ISP timelines.
- Medical Plan of Care and Nursing Care Plan are maintained and modified quarterly.
- Episodic Case Management for hospital admissions and discharges.
- Assists in the review of referrals and the intake process for new residents. Conducts a comprehensive nursing assessment and develops the medical record, medical training, specialized protocols and data collection collaboratively with the individual, family, and caregivers.
- Must be fully licensed as a Registered Nurse RN in Massachusetts and must maintain licensure from the Massachusetts Board of Registration in Nursing. Minimum three years of nursing experience and three years of experience in the direct care of the elderly or disabled adults are required.
- Must drive to various locations for home visiting purposes and medical appointments.
- Light physical effort might be required.
- Must have extensive knowledge of MAP (medication administration program). Prior experience developing training and ensuring compliance with MAP.
