Per Diem

Summary

The Per Diem Physical Therapist provides skilled hospice care services to adult and geriatric patients across a variety of settings, including private homes, assisted living communities, and inpatient facilities. As a key member of the interdisciplinary care team, the Physical Therapist delivers patient‑and‑family‑centered care that promotes comfort, safety, mobility, and dignity at end of life. This role supports patients in remaining in their home setting whenever possible while receiving high‑quality, compassionate care.

• Minimum of one year of clinical experience as a licensed Physical Therapist required.
• Graduate of an accredited physical therapy program.
• Home health or hospice experience strongly preferred; or an equivalent combination of education and experience that provides the necessary skills to perform the role.
• Ability to work independently and manage time effectively in a field‑based environment.
• Strong clinical assessment, problem‑solving, and organizational skills.
• Excellent interpersonal and communication skills.
• Proficiency with electronic medical records (EMR) systems preferred.
• Valid driver’s license and reliable transportation required.

Pay Range

$58—$58 USD
