Summary
The Program Manager oversees the daily operation of a Day Habilitation program for people with disabilities. Responsibilities include supervision of staff (hiring, discipline, and training of staff members), supervising staff activities, coordinating staff activities to meet the program objectives, and overseeing member services.
Schedule
Monday-Friday 8:00am-4:00pm (40 hours per week)
Essential Duties & Responsibilities Maintain appropriate certifications (i.e. First Aid, CPR, OSHA, Fire Safety, Human Rights, etc.) and ensure that all staff maintain their certifications.Provide leadership to staff by creating a cohesive team environment.Oversee the administering and interpreting of assessments to determine each individual's skill level on an annual basis.Oversee and assure that all documentation is completed within timelines and meets Mass Health regulations.Supervises and manages all data collection.Assist Clinical Team and staff with referrals and development of services within the Day Habilitation site.Maintain budget for the Day Habilitation program within allowed spending.Maintain communication and collaborative relationships with other service providers and DDS.Serve as liaison between Day Services, Residential and family for individuals.Maintain sites according to DDS, Mass Health, CARF and Agency standards.Ensure the staffing needs of the program are met. This includes coordination of work schedules, hiring of new staff, and coordinating the use of per-diem and agency relief staff, as needed.Positively manage staff by conducting monthly supervisions with each staff and team meetings, supporting, coaching and mentoring staff to develop skills and abilities, and providing recognition opportunities for staff.Provide timely Performance Evaluations. Ensure performance standards are met, and proactively address performance concerns, working with program management and Human Resources, as appropriate.Ensure self and employees maintain certifications (such as CPR/1st Aid, MAP, PPS and PABC, as required) and complete all other training requirements of Bridgewell and the program's funding sources.Work collaboratively with clinical and nursing teams to ensure the behavioral and medical needs of the people served are met. Collaborate with all service providers, ensuring communication and documentation is clear regarding the coordination and management of each person's needs.Manage the technology needs of the program, including employee time and attendance, and data management/electronic health record systems. Ensure information is tracked as required, and system needs and issues are raised to the IT Department or Payroll Department, as appropriate.Ensure the confidentiality of Personal Health Information, records of people served, and other related information is in compliance with HIPAA, and other state or federal mandates.Demonstrate the core philosophy of universal supports by embracing Bridgewell's core values: dignity, respect, caring, integrity, safety and wellnessOther duties as assigned.Required Education/Experience
Bachelor's degree in a health-related field with a minimum of 3-years relevant healthcare experience and minimum of 2 years in a supervisory role within a relevant healthcare setting. ORHigh School diploma with a minimum of 6 years of relevant healthcare experience with 3 of those years in a supervisory role.Working experience in understanding the clinical needs of the people served is required.A valid US driver's license and one year of driving experience is required.Knowledge/Skills
Knowledge of Day Habilitation regulations and CARF standards.Knowledge of Behavior Management and Human Rights.Excellent organizational skills, problem solving and communications skills.Ability to lead and supervise staff to performance standards.Conflict Resolution and problem-solving skills.Ability to work as a cohesive team member with other inter-disciplinary team members…
