  • Participate in comprehensive physical health assessments of people served by Vinfen
  • Develop and implement person-centered treatment care plans
  • Provide nursing care and assistance with activities of daily living, including diabetes care, wound care, GT/JT care, and nutritional support
  • Monitor clients and perform assessments when there is a change in status
  • Provide skilled nursing interventions
  • Administer medications and treatments as ordered by primary care providers under applicable MAP guidelines
  • Provide transportation and accompany individuals to medical, dental, and other appointments
  • Ensure nutritional needs, medical supplies, pharmacological supplies, and equipment are managed safely and timely
  • Utilize behavioral strategies and prescribed clinical plans
  • Supervise C.N.A.s and ensure care is timely, accurate, person-centered, and aligned with ISP goals
  • Provide nursing education and training to C.N.A.s
  • Communicate with program leadership, primary care providers, families, guardians, agencies, employees, and community contacts
  • Use emergency and on-call systems when appropriate
  • Document nursing care, treatments, medication administration, shift notes, communication, treatment plans, medical records, and incident reports
  • Maintain a safe, clean, comfortable, welcoming, and regulatory-compliant environment
  • Maintain client medical equipment and supplies, including wheelchairs, walkers, Hoyer lifts, and hospital beds
  • Support community outings and events and respect individual preferences
  • Advocate for people served and collaborate with Vinfen departments and community providers
  • Participate in treatment plan, ISP, and other planning meetings
  • Attend Vinfen trainings, conferences, and educational opportunities
  • Comply with accreditation, licensing, fire, and safety requirements
  • Perform other assigned duties
Requirements
  • High school diploma or GED required
  • Current L.P.N. license in state of service required
  • Bachelor's degree in Social Work, Psychology or related field preferred; experience may substitute for education
  • 1-2 years supervised practical experience, including internships, in a clinical or rehabilitation setting preferred/helpful
  • Experience working with people with psychiatric conditions and/or co-occurring disorders strongly preferred
  • Strong time management and writing skills
  • At least 21 years of age
  • Current, valid US driver's license held for at least 6 months; another section requires at least one year, plus adequate insurance if using a personal vehicle and a safe driving record
  • Must become CPR and First Aid certified within 2 weeks of employment
  • Must become MAP certified within 150 days of employment
  • CPR required within two weeks of hire
  • Safety Care required within 90 days of hire
  • Must successfully pass CORI, reference, multi-state background, driving record, and driver screening checks
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds, and assist clients with activities of daily living
  • Ability to operate a computer and office equipment
  • Knowledge of LPN practice standards, intervention techniques, developmental disabilities, assessment practices, documentation standards, community health and mental health services, and personal computer applications
  • Ability to supervise and mentor C.N.A.s
  • Ability to make independent judgments and decisions and work professionally and confidentially
