Physical Therapist or Physical Therapist Assistant

We are seeking a compassionate Physical Therapist Assistant to join our Healthcare & Medical Services team. Responsibilities

• Provide high-quality rehab services to seniors in skilled nursing and long-term care settings.
• Deliver individualized treatments under the supervision of a Physical Therapist, including therapeutic exercises, gait and balance training, and functional mobility enhancement.
• Collaborate with an interdisciplinary team to develop and implement patient care plans.
• Document patient progress accurately in the EMR system.
• Educate patients and caregivers on treatment plans and safety techniques.

• Strong interpersonal skills with a compassionate approach.
• Knowledge of therapeutic exercises, mobility training, and safety protocols.
• Ability to work effectively within a team environment.
• Attention to detail for accurate documentation.

• Assist in improving patients' mobility, safety, and independence.
• Support the overall goal of providing patient-centered, dignified care in diverse post-acute settings.
