The Licensed Practical Nurse (LPN) plays a key role in supporting the health and well-being of residents in Assisted Living and Memory Care. This position is responsible for clinical assessments, medication administration, care planning, and ensuring residents receive high-quality, individualized care while maintaining dignity and independence.

• Administer medications and treatments in accordance with physician orders and state regulations
• Conduct resident assessments and assist in care planning and updates
• Monitor resident conditions and communicate changes to the care team
• Ensure accurate and timely documentation of nursing services
• Support and supervise caregivers and med techs as needed
• Promote a safe, clean, and compliant care environment
• Build strong relationships with residents and families

• Active Licensed Practical Nurse (LPN) license in the state of Massachusetts required
• Previous assisted living, memory care, or long-term care experience preferred
• Strong knowledge of medication administration and clinical best practices
• Excellent communication and teamwork skills
• Ability to work in a fast-paced environment while maintaining attention to detail
• Compassionate, dependable, and resident-focused approach

• Ability to stand, walk, bend, and assist residents for extended periods
• Ability to lift and support residents as needed in accordance with safety guidelines
• Exposure to healthcare environments, including communicable diseases
