The Licensed Practical Nurse (LPN) plays a key role in supporting the health and well-being of residents in Assisted Living and Memory Care. This position is responsible for clinical assessments, medication administration, care planning, and ensuring residents receive high-quality, individualized care while maintaining dignity and independence.
Key Responsibilities- Administer medications and treatments in accordance with physician orders and state regulations
- Conduct resident assessments and assist in care planning and updates
- Monitor resident conditions and communicate changes to the care team
- Ensure accurate and timely documentation of nursing services
- Support and supervise caregivers and med techs as needed
- Promote a safe, clean, and compliant care environment
- Build strong relationships with residents and families
- Active Licensed Practical Nurse (LPN) license in the state of Massachusetts required
- Previous assisted living, memory care, or long-term care experience preferred
- Strong knowledge of medication administration and clinical best practices
- Excellent communication and teamwork skills
- Ability to work in a fast-paced environment while maintaining attention to detail
- Compassionate, dependable, and resident-focused approach
- Ability to stand, walk, bend, and assist residents for extended periods
- Ability to lift and support residents as needed in accordance with safety guidelines
- Exposure to healthcare environments, including communicable diseases
