Job DescriptionThe Hospice RN Float plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation. They interact effectively with patients, significant others, and other interdisciplinary team members while maintaining standards of professional nursing and clinical competency. The role includes providing palliative and hospice care through assessment, teaching, counseling, and direct care to clients and/or family members, and coordinating care with community resources and team members.
A predictable location cannot be assumed; travel is required throughout the BILH at Home service area as needed.
- Manage care of patients, coordinate all services, and facilitate communication between all providers.
- Provide skilled nursing care and prescribed treatments to patients and families in their home.
- Maintain productivity expectations.
- Provide documentation of services using a problem‑oriented record.
- Plan for continuity of care with nursing team members, patient, family, and other disciplines.
- Use opportunities for teaching nursing care, comfort measures, and health practices to the patient, families, and others.
- Instruct and supervise care given by the Home Health Aides.
- Communicate and coordinate with other health and social agency personnel to assist in planning, implementing, and evaluating service to the patient and family.
- Participate in bi‑weekly hospice case conferences or as required.
- Complete Hospice Hope Tool time points and accept accountability for patient satisfaction scores and quality of care outcome measures.
- Complete and document an initial assessment of patient and family to determine hospice care needs, including a complete physical assessment and history of current and previous illnesses. Develop a patient‑specific plan of care with goals based on nursing diagnosis and include pain and symptom management. Include the patient and family in the planning process.
- Initiate the plan of care, making revisions as patient status and needs change.
- Administer medications and treatments as prescribed by the physician.
- Counsel the patient and family on meeting patient needs related to physical, emotional, and spiritual needs.
- Respect and support patient and family’s cultural beliefs, values, health goals, and treatment preferences.
- Provide information to the patient and family regarding the dying process and expectations.
- Provide instruction regarding medication actions, side effects, dosage, route, and frequency.
- Instruct, supervise, and evaluate hospice health aide care. Support supervision of care provided by the hospice aide every 14 days and as needed.
- Assist in managing hospice benefit periods according to evolving patient needs and continued eligibility for services.
- Perform liaison visits and hospice informational visits to patients in the community and facilities to discuss hospice and palliative care services.
- Communicate with the physician, Hospice RN Case Manager, and clinical manager regarding patient needs and report any changes in the patient’s condition; obtain and receive physician orders as required.
- Attend and contribute to IDT meetings every two weeks at a minimum and when needed.
- Communicate and coordinate with the team LPN as appropriate.
- Collaborate, communicate, and cooperate with other health care providers to ensure quality end‑of‑life care.
- Demonstrate proficiency in Hope Tool completion, including HUV and symptom follow‑up visits.
- Ensure documentation supports hospice eligibility.
- Ensure documentation supports that the patient meets ongoing hospice criteria.
- Complete documentation in accordance with agency timeliness policies.
- Maintain and update clinical knowledge and skills based on current nursing practice.
- Adhere to departmental requirements for required training.
- Identify specific learning needs and goals and collaborate with nursing leadership in developing a plan to meet them.
