Lead Triage Nurse
MFM Health is seeking a full-time, experienced, patient-centered Lead Triage Nurse to provide clinical leadership and operational support for our Triage Nurse team. This position includes rotating coverage among our Middleton, Danvers, and Manchester-by-the-Sea locations, offering the opportunity to collaborate with providers and staff across multiple practice sites. Working closely with the Manager of Clinical Support & Care Continuity Operations, the Lead Triage Nurse helps coordinate the day-to-day activities of the department, ensuring efficient operations, adherence to clinical standards, and the delivery of safe, timely, and patient-centered care.

- Support the Manager of Clinical Support & Care Continuity Operations in the daily oversight of the Nursing and Triage department.
- Coordinate daily triage operations to ensure timely and efficient patient care.
- Monitor department workflow throughout the day and adjust assignments based on staffing levels, patient volume, and operational needs.
- Identify workflow challenges and recommend process improvements.
- Assist in implementing departmental initiatives and operational goals.
- Assist with maintaining appropriate daily staffing coverage.
- Coordinate triage assignments and adjust schedule as necessary due to staffing changes, call-outs, or fluctuating patient volume.
- Support adherence to departmental scheduling, attendance, and PTO policies.
- Communicate staffing concerns promptly to the Manager of Clinical Support & Care Continuity Operations.
- Coordinate weekly and daily triage assignments to ensure equitable workload distribution.
- Monitor triage performance to ensure calls, patient messages, and clinical tasks are managed efficiently and in accordance with departmental standards.
- Ensure appropriate triage coverage across all clinical teams.
- Reinforce established triage workflows and elevate ongoing operational concerns when appropriate.
- Monitor nurse visit workflows to ensure consistency with organizational standards.
- Review and approve nurse visits for accuracy, completeness, and appropriate documentation.
- Assist in developing and improving standardized nurse visit protocols.
- Promote consistent, evidence-based nursing practices throughout the department.
- Maintain comprehensive vaccine reference resources for nursing staff.
- Ensure vaccine administration guidelines remain current and consistent with CDC, ACIP, state and organizational recommendations.
- Maintain guidance regarding:
- Vaccine schedules
- Age indications
- Administration intervals
- Needle selection (length and gauge)
- Frequency and spacing requirements
- Serve as a clinical resource for vaccine-related questions and updates.
- Serve as the primary point of contact for day-to-day clinical questions and workflow concerns.
- Provide guidance, coaching, and mentoring to nursing and triage staff.
- Support onboarding and ongoing training of new nursing team members.
- Assist the Manager of Clinical Support & Care Continuity Operations with leadership responsibilities during absences or as delegated.
- Promote professionalism, accountability, teamwork, and respectful communication.
- Collaborate with providers, clinical leadership, and department leaders to ensure coordinated patient care.
- Promote effective communication between departments.
- Assist in resolving workflow concerns that impact patient care or operational efficiency.
- Support organization-wide initiatives designed to improve clinical operations and patient experience.
- Foster a culture of professionalism, accountability, respect, and teamwork.
- Address minor workflow or behavioral concerns through coaching and real-time feedback.
- Escalate significant performance or conduct concerns to the Manager of Clinical Support & Care Continuity Operations.
- Encourage positive collaboration among nursing staff and across departments.
