MDS Coordinator LPN About the Role

As an MDS Coordinator LPN at Grandview Nursing & Rehabilitation, you will be responsible for ensuring that our residents receive the highest level of care possible. You will work closely with our interdisciplinary team to develop and implement care plans, monitor resident progress, and maintain accurate and detailed records. Your attention to detail, organizational skills, and ability to communicate effectively will be essential in this role.

• Conduct initial and ongoing assessments of residents to identify needs and develop individualized care plans
• Collaborate with healthcare team members to ensure comprehensive care and services are provided
• Maintain accurate and detailed records of assessments, care plans, and resident progress
• Develop and implement strategies to improve resident outcomes and reduce readmissions
• Participate in quality improvement initiatives and maintain compliance with regulatory guidelines
• Assist with resident admissions, discharges, and transfers
• Maintain a clean and organized workspace, adhering to infection control protocols

• Licensed Practical Nurse (LPN) with current licensure
• 1+ years of experience in MDS
• Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities
• Strong communication, interpersonal, and organizational skills
• Familiarity with MDS regulations and guidelines
• Basic knowledge of electronic medical records and other software applications
