• Provide direct patient care, including administering medications, wound care, and vital sign monitoring

• Develop and implement individualized care plans in collaboration with our healthcare team

• Assist with ADLs, including bathing, dressing, and grooming

• Maintain accurate and detailed records of patient care and treatment

• Collaborate with the interdisciplinary team to develop and implement discharge plans

• Provide education and support to patients and their families on disease management, self‑care, and other health‑related topics
