Registered Nurse - Hospice
The Care Team Hospice is seeking a compassionate and patient-focused Hospice RN Case Manager to join our team. This is a rewarding opportunity for a nurse who is passionate about providing exceptional end-of-life care while building meaningful relationships with patients and families.
Schedule, Hours & Territory- 2nd Shift | 2p-10p
- Serving patients within approximately a one-hour of Branch

- Active RN License required
- Hospice nursing experience preferred
- Experience with Homecare Homebase (HCHB) preferred
- Strong assessment, communication, and care coordination skills
- Monday-Friday, 8:00 AM - 4:30 PM
- On-call opportunities available with additional compensation
- Some locations offer dedicated on-call teams, resulting in backup coverage only
- Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards, physician orders, and individualized plans of care.
- Complete scheduled visits, admissions, and clinical interventions while maintaining accurate and timely documentation in Homecare Homebase (HCHB).
- Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
- Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
- Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.
