Licensed Practical Nurse

Liberty Care & Rehabilitation Center is seeking a compassionate Licensed Practical Nurse to provide direct bedside care to residents in our skilled nursing and rehab community. The LPN will administer medications and treatments, monitor vital signs and resident conditions, document accurately in electronic records, and coordinate with RNs, physicians, and therapists to support individualized care plans. You will educate residents and families, support ADLs, and respond promptly to changes in condition.

Responsibilities

• Provide direct bedside nursing care to residents in a skilled nursing and rehab setting
• Administer medications and treatments per physician orders and facility protocols
• Monitor, record, and report vital signs and changes in resident condition
• Document care and observations accurately in electronic medical records
• Collaborate with RNs, physicians, therapists, and CNAs on individualized care plans
• Assist residents with activities of daily living and promote comfort and safety
• Educate residents and families on care plans, medications, and health needs
• Follow infection control, safety, and regulatory guidelines at all times
• Participate in shift reports, care conferences, and quality improvement efforts
• Support a resident-centered culture of dignity, respect, and compassion
